Refusal to Vaccinate Forms
Please see the article on my website about these and the letter I helped a MI mom draft to state officials there about the MI vaccine exemption form that has similar language below. I think these forms raise serious ethical and possibly legal problems:

Some simply cross out the parts they disagree with, to avoid agreeing to something that might be used against them later and that they disagree with. If the entity requiring the form will accept that, it's a reasonable thing to do. But the use of these forms is fundamentally wrong, and should be contested--perhaps not by individuals confrontationally (it's usually better to go with the flow if you can), but to state medical boards, state health departments, etc.--and to state legislators to enact laws prohibiting the use of these unethical and possibly unlawful forms.
